REQUIRED CREDENTIALS

Applicants are required to submit all of the following documents. All applications must be typed.

* HSHPS/CDC Fellowship Program Application Form A-2

* Personal Statement —Applicants are required to submit a Personal Statement between 500-750 words. This statement should describe: the reasons why you are interested in a mentored training program; your specific interests and experiences in the area of Hispanic health; any special skills or experiences that may relate to an HSHPS fellowship; and your future career plans.

* Curriculum Vitae (C.V.) —Applicants are required to submit a complete c.v., detailing their academic and employment histories, as well as listing honors and awards received.

* Transcripts —Applicants are required to submit official transcripts for each college or university attended. The transcripts should be mailed directly to HSHPS.

* Letters of Recommendation (2) —At least one letter must be submitted by a full-time faculty of a HSHPS member institution in your field of study who is familiar with your academic work and HSHPS. The second letter must be completed by a professional or academic supervisor/mentor who is familiar with your professional aspirations and potential. Individuals submitting letters of recommendation should use their respective institution/organization letterhead. The letters should be mailed directly to HSHPS.
